Complete applicable sections of the MDS, Resident Assessment Protocols , and care plans in a timely manner for appropriate residents on assigned community according to Federal guidelines. MDS Nurses will ensure timely completion of Minimum Data Sets and care plans for all Healthcare residents and patients. Responsible for the overall development of staff clinical competencies, monitoring and coordination of education programs in healthcare including state required educational in-services. Care Attendants will provide assistance to Evergreen Place residents participating in the assisted living program. At Greencroft Healthcare there are 228 residents that are on Medicare/Medicaid.

DiningTeam Leaders are responsible to serve food in a safe and sanitary manner, provide leadership for dining attendants, and promote team effort within the department. Previous nursing experience required; five years preferred. Responsible for maintaining training records to include training topics and desired outcomes, attendance. Regularly review training compliance, to include Relias, and work with administrator/designee on resolving issues of team members who are not in compliant with required training. At least 1-2 years’ experience in long-term care billing and bookkeeping required.

Provide changes of clothing, briefs, bedding, etc as needed in order to keep the client and their living area clean. Assist with dressing/undressing as needed, per Care Plan. Interact with clients and team members in a respectful, adult manner. Monitor all activity assuring that state regulations are adhered to through resident clinical care and documentation. Two years teaching experience in Long Term Care is preferred. Previous experience in presentations and educational settings preferred.
